Prescription drug coverage: This plan has its own formulary list. Tier 1 (Generic Drugs): 15 copay. Tier 2 (formulary band name drugs): $35 copay after the $2000 annual prescription drug deductible per member is met. Specialty 25% coinsurance up to a $2,500 annual prescription drug out-of-pocket maximum (the most you'll have to pay), for network only and in addition to the $2,000 annual deductible.
